Our Mission

Our patients have a broad range of life-limiting illnesses including, but not limited to, various forms of cancer, stroke, and heart/liver/ lung/ kidney /ALS and Alzheimer's disease, as well as AIDS.

When medical treatments can no longer cure a disease, we provide an individualized program of physical, emotional, spiritual and practical care for persons in the last stages of a life-limiting illness, with an emphasis on symptom management (control of pain and reduction of anxiety) within familiar surroundings, rather than curative treatment. Our palliative care program focuses on quality of life (comfort and dignity) for terminally ill people and is centered on both the patient and his/her family.

Services

Our services are provided wherever people with life-limiting illnesses reside including their homes, In-patient facilities, hospitals, nursing homes, assisted living facilities and residential care facilities.

Each patient is cared for by an Interdisciplinary Hospice Team (IDT) comprised of experts who are trained, certified and/or licensed as Physicians, Nurses, Social Workers, Counselors, and other health related fields, and are on-call 24 hours a day, 7 days a week. This team designs a Plan of Care for each patient and implements it in conjunction with the patient and his/her family and/or caregivers.

Caregivers consistently rate us in the top 1% of all hospice agencies for “Patient Satisfaction” based on the care and services we provided their loved one.

Our staff is multilingual. We are proficient in English, Spanish, Korean and Tagalog. If you need the services of an interpreter for a specific language, please feel free to ask us. We will provide you with an interpreter at no cost to you.

Our administrative office is open from 9:00 AM to 5:30 PM Monday through Friday; However, if you have needs after hours, CALL US at 562-291-2052. An on-call team consisting of a Nurse, a Social Worker, a Chaplain, or an Administrator will be available via phone within a few minutes.

Our on-call team is available at all times to:

  1. Make a visit when needed
  2. Respond to a sudden change of condition
  3. Answer questions
  4. Order medication, oxygen, or medical equipment
  5. Provide emotional or spiritual support

We are accredited by the Joint Commission to provide hospice care in California
